Accepting New Patients.
New patients please arrive 15 minutes prior to scheduled appointment time to complete registration and insurance forms.
Please bring insurance card and insurance copay to each appointment.

Phone messages are handled as follows:

• Prescription refills: Please have your medication information and pharmacy phone number available when you call in. Allow 24 hours for the refill process.
• Urgent messages will be expedited. Please let the receptionist know if the message is urgent. Women in labor, excessive bleeding, & pain need priority.
• Non urgent calls and questions, will receive a return call within 24 hours.
• Please be sure to leave a number / numbers where you can be reached.
• Test results will be reviewed by your provider and you will be notified of results in a timely manner.
